Patient card/medical ID card
When visiting the hospital, a patient card is required. If you have not previously been examined or treated at the Maastricht UMC+/azM, please register for a patient card before visiting the outpatient clinic. This can be done at the Patient Registration Desk. The desk is located on the ground floor in the ‘Poliplein’ and is open on weekdays from 08.00 to 17.00. Please provide your insurance details (an insurance card or policy document) and a valid ID. The staff will enter your personal details and give you a patient card with your patient number (this is the SAP number).
Are you planning to move, do you have a new phone, or do you have a different primary care physician or insurance provider? Please inform the Patient Registration Desk of these changes as soon as possible.
