You have the right to see your medical records. If you wish to see your records, please discuss this with your doctor or the nurse at your ward.
Requests for copies of medical records
Do you wish to receive a copy of your medical records? For reasons of safety, requests for copies have to be made in writing. You can download further information and an application form via the patient information page 'Your medical records'. You can also contact the Patient Information Centre (telephone: +31-(0)43-387 4567) and ask for an application form. Or you can collect the form yourself from the Patient Information Centre, which is located in the Serrehal. (interactive map).
The application form is only available in Dutch, but our staff at the Patient Information Centre will gladly help you fill it out.
The following charges apply to sending you copies of records by postal mail:
- Up to 100 pages: € 4.50100 pages or more: € 22.50
- Data that are difficult to access: € 22.50 per document
- X-rays: € 10.00 per CD-ROM
- Copies from Medical Photography: € 2.50
- Registered mail surcharge: € 8.15
Copies of X-rays
Copies of X-rays can be collected from the 'copy desk' of the Radiology department at level 3, waiting area 2. The desk is open every day from 8.00 a.m. to 12.00 p.m. (noon) and from 1.30 p.m. to 4.45 p.m. For privacy reasons, only the patient (or their legal representative) can request a copy of the results. We will ask you to prove your identity by means of a valid identification document, i.e. your passport, ID card or driving licence. The cost (€10.00) can be paid by debit card when collecting the CD/DVD.
