Patient ID card

In our hospital you will need a patient ID card. If you are a patient for the first time, please visit our Patient Registration Office to get a patient ID card made. You will also need to register your citizen service number. To register you will need your insurance details (an insurance card or policy document) and a valid ID.

The Office Patient Registration desk is located on the 1st floor and is open on weekdays from 8:00 a.m. to 5:00 p.m.

Application overview hospital visit

Social security number
