Patient card/medical ID card

Always bring your patient card (medical ID card) and a valid ID

When visiting the hospital, a patient card and a valid ID is always required. If you have not previously been examined or treated at the Maastricht UMC+/azM, please register for a patient card before visiting the outpatient clinic. This can be done at the Patient Registration Desk. The desk is located on the ground floor in the ‘Poliplein’ and is open on weekdays from 08.00 to 17.00. The desk can be reached by phone at +31- (0)43-387 68 33 / email: pr@mumc.nl.

Please provide your insurance details (an insurance card or policy document) and a valid ID. The staff will enter your personal details and give you a patient card with your patient number (this is the SAP number).

Card changes

Are you planning to move, do you have a new phone, or do you have a different primary care physician or insurance provider? Please inform the Patient Registration Desk of these changes as soon as possible. You can submit your changes by email via pr@mumc.nl, at the Patient Registration Desk or by phone at +31-(0)43-387 68 33.
